The other two letters should be professional references who can speak to your past performance in processing new information, and your potential performance in a graduate program.
• One Set of Official Transcripts
• Official GRE Scores
• Official TOEFL or IELTS Scores (International applicants only)
• SOPHAS Application fee of $95.00 or more

2. Submit an online application to UCLA Graduate Admission (Part A) at www.gradadmissions.ucla.edu and the following application materials:

• UCLA School of Public Health Supplemental Application
• One set of Official Transcripts
• Official GRE Scores
• Official TOEFL or IELTS Scores (International applicants only)
• UCLA Graduate Division Application Fee of $60.00 (U.S. Citizens and Permanent Residents) or $80 (for all other applicants).
