Application Checklist

1. Submit an online application to SOPHAS at www.sophas.org and the following application materials:

• Statement of Purpose and Objectives
• Resume or Curriculum Vitae (CV)
• Three Letters of Recommendation - two from former professors and one from an employer. If applicant has been out of school for over five years, three letters from employers are acceptable.
• One Set of Official Transcripts
• Official GRE Scores
• Official TOEFL or IELTS Scores (International applicants only)
• SOPHAS Application fee of $95.00 or more

2. Submit an online application to UCLA Graduate Admission (Part A) at www.gradadmissions.ucla.edu and the following application materials:

• UCLA School of Public Health Supplemental Application
• One set of Official Transcripts
• Official GRE Scores
• Official TOEFL or IELTS Scores (International applicants only)
• UCLA Graduate Division Application Fee of $60.00 (U.S. Citizens and Permanent Residents) or $80 (for all other applicants).
